To correctly set up a Form, you will need several pieces of information.  You will want to request completed samples of all work types (patient information omitted).  An in-depth analysis of these samples will help you to create a Form identical to what your customer is used to seeing.  Make note of the following:

 

 Font

 Font size

 Margins

 Headers - are headers on the 1st page only, different 1st and 2nd page headers?

 Footers - are footers on the 1st page only, different 1st and 2nd page footers?

 Graphics

 Mandatory headings, i.e., CHIEF COMPLAINT, PHYSICAL EXAM, etc.

 Turn around time for specific work types

 Patient and Provider information that needs to be on the final document
