If you encounter any part of the dictation that cannot be transcribed, insert an Incomplete Marker. The Incomplete Marker can be used instead of inserting blanks (_____) to the document. Inserting an Incomplete Marker will route your job directly to QA.
The Incomplete Marker tags your document with the precise audio moment of the ChartNet Voice Player at the time of insertion. This marks the spot of the blank in the document, allowing QA to quickly move the audio to the location in question.
An Incomplete Marker will appear in your document as <INC 00:00:00>, where the 00:00:00 denotes the HOURS:MINUTES:SECONDS of the audio location at the time of insertion. See the image below for an example of an Incomplete Marker in the ChartNet Editor.

The steps to insert an Incomplete Marker are the same in both the ChartNet and M*Modal Editors, although the markers appear differently between the two editors.
You may insert as many Incomplete Markers as required, although it is recommended to add an Internal Note to your report to explain a significant number of Incomplete Markers. For example, if the audio file of your job is distorted or corrupted, you may wish to leave an Internal Note explaining the situation to QA instead of leaving multiple Incomplete Markers. Your manager will explain the proper procedure for inserting Incomplete Markers.
